What you'll need to get started...

There are a few forms I like to have you fill out and bring to our first session. The first is a general form with a little information about you. The second is a health appraisal form, which helps me pinpoint various health issues you may be experiencing. The third is a disclaimer form, and the last form is a general guideline for the food journal.

While I do not want the forms to be a hindrance to you actually making or coming to an appointment, they are important and very helpful. So please attempt to do them, and be as thorough as possible. Remember, this is an investment in your health!
